Formatting notes:
  • Many browsers allow you to customize the header and footer for each page.  For example, with Mozilla, select the menus File -> Page Setup... -> Margins & Headers/Footers.  The headers and footer can be turned off to avoid that web page look when they are printed.
  • Your browser may also allow you to adjust the page orientation and the margins.  The tables were written to adjust to the size of the printable area.  With Mozilla, this is done in the same menu that's used to change the header and footer.
